The treatment period begins on Day 1 and continues through the hospital stay, followed by a 90-day plus 2-week follow-up after discharge. The trial uses a triple-blind design to ensure unbiased results. During the approximately 90-day study, patients will undergo physical exams, laboratory tests, biomarker collections, ECGs, chest X-rays or CT scans, and complete questionnaires. Researchers will monitor ventilatory status, oxygenation levels, survival rates, and functional status using WHO Ordinal and Karnofsky scales. Resource use such as days on ventilators, ICU stays, and hospital length of stay will also be tracked, along with safety assessments for adverse events throughout the study period.

- Adults aged 18 years or older
- Hospitalized with moderate or severe Acute Respiratory Distress Syndrome (ARDS) based on PaO2/FiO2 or SpO2/FiO2 at randomization

- Known allergy or hypersensitivity to paridiprubart or its excipients, including polysorbate 80

Participants are screened for eligibility to participate in the trial.
1 screening and enrollment visit (in-person)
Duration - From Day 1 through hospital discharge
Participants receive a single IV dose of paridiprubart or placebo starting on Day 1 while hospitalized. They continue to receive standard treatments for ARDS as per the investigator.
Daily assessments during hospitalization
Duration - From hospital discharge through approximately Day 90 plus 2 weeks
Participants are followed after hospital discharge to monitor recovery and collect outcome data including functional status and adverse events.
Multiple visits for physical exams, laboratory assessments, imaging, and questionnaires through Day 90
