This study supports a program to train community staff, improve referral pathways, and optimize patient care while reducing emergency hospital admissions.

Not Eligible

You will not qualify if you...

  • Clinical diagnosis of Alzheimer's Disease or Vascular Dementia
  • Palliative Care patients.

Diagnostic Evaluation

Participants undergo a cardiac ultrasound scan using an abbreviated echocardiogram protocol to detect valvular heart disease and left ventricular systolic dysfunction.

Duration - 12 months

Participants are observed over 12 months to assess the presence and diagnostic accuracy of heart valve disease and left ventricular systolic dysfunction.
