What this Trial Is About
Researchers are evaluating eGAPcare, a telehealth version of the GAPcare fall prevention program, in older adults visiting community emergency departments. The study aims to find out if physical therapy and pharmacy consultations can be effectively delivered via telehealth during an emergency visit, and if this approach reduces the number of recurrent falls within six months. Participants will receive telehealth physical therapy and pharmacy consultation during their emergency department visit. After this initial visit, they will take part in follow-up telephone calls for six months. The study focuses on using telehealth to make fall prevention care more accessible and convenient for older adults. Throughout the six months, participants will complete questionnaires administered by study staff. Researchers will track falls reported by participants or caregivers as well as falls and healthcare use recorded in electronic health records. This ongoing monitoring helps evaluate the effectiveness and feasibility of the telehealth intervention over time.
