What this Trial Is About

This trial investigates adults undergoing elective cardiac surgery with cardiopulmonary bypass, assessing whether a nonintubated supraglottic airway anesthesia method with early spontaneous breathing reduces postoperative lung complications and speeds recovery compared to conventional endotracheal fast-track anesthesia. It aims to improve outcomes such as pulmonary complications, respiratory support needs, and intensive care unit time. Participants will be randomly assigned to either the nonintubated anesthesia group, using a supraglottic airway without routine intubation and early breathing restoration when possible, or the conventional group using endotracheal intubation and lung-protective ventilation. Both groups receive standardized perioperative care with objective criteria guiding airway removal and ICU discharge. Safety measures allow switching to intubation or increased respiratorycirculatory support if needed. Participants are followed from screening through their hospital stay, with assessments at 30 days and up to 1 year post-surgery. Data collected include pulmonary complications, respiratory support duration, ICU stay length, organ complications, safety events, recovery measures, rehospitalization, and survival. An independent committee will review key pulmonary outcomes using predefined criteria.
