Participants in this study include clinical and leadership staff involved in heart failure care with at least six months of experience. The study uses interviews, observations, and surveys to gather information about how to best implement the program. Researchers will analyze qualitative data to identify themes that guide the adaptation and implementation process. The goal is to make the ADAPT program practical and helpful for more patients living with heart failure in community settings. The study is expected to run until September 2028.

You may qualify if you...

  • Outpatient clinician or staff member such as physician, physician assistant, nurse practitioner, registered nurse, social worker, psychologist, or medical assistant
  • Healthcare leadership personnel such as Chief Nursing Officer or Clinic Medical Director
  • At least 6 months of experience caring for patients with heart failure
  • 18 years or older

History of severe allergic reactions to study medication Currently pregnant or breastfeeding Recent participation in another clinical trial within the last 30 days Presence of uncontrolled medical conditions that could affect safety
