People living with heart failure often experience difficulty in daily life due to the heart's reduced ability to pump blood, leading to frequent hospital visits and ongoing symptoms. The ADAPT program, which stands for "Advancing Symptom Alleviation with Palliative Treatment," was developed to provide additional support beyond usual heart failure care. It involves nurses and social workers making weekly calls to help manage symptoms, provide coping tools, and keep the patient's healthcare providers involved. Previous research showed ADAPT improved quality of life and reduced depression, anxiety, and heart failure symptoms. This study aims to see if the ADAPT program can be successfully adapted and used in community healthcare settings outside of research. The study will work with healthcare providers, including physicians, nurses, social workers, and leadership staff, to understand how to adjust the ADAPT program for use in various settings such as primary care or heart failure clinics. Using feedback from interviews, clinic observations, and surveys, the team will develop simple materials and training resources to help these providers easily offer the ADAPT support. This will prepare for future testing of the adapted program in real-world clinical environments. Participants in this study include clinical and leadership staff involved in heart failure care with at least six months of experience. The study uses interviews, observations, and surveys to gather information about how to best implement the program. Researchers will analyze qualitative data to identify themes that guide the adaptation and implementation process. The goal is to make the ADAPT program practical and helpful for more patients living with heart failure in community settings. The study is expected to run until September 2028.
