Frailty is a condition that mainly affects older adults and those undergoing long-term treatments like cancer or chronic pain. It increases the risk of disability and complications after surgery. With an aging population in Latin America and Chile, frailty is becoming more common among surgical patients, yet preoperative frailty assessments are rarely done in Chile due to logistical and training challenges. This study focuses on a prehabilitation program called "Prepare for Your Surgery," designed to improve patient resilience before elective surgery by reducing frailty and complications. The program includes multiple interventions starting 4 to 6 weeks before surgery. These involve nursing assessments and screenings using various frailty, cognitive, and functional tests; a medical consultation to optimize health and manage comorbidities; 8 to 10 physiotherapy sessions; 1 to 2 nutritional interventions; and additional follow-up via phone or email if needed. Hospital data on surgical complications, length of stay, and care level will be collected during the hospital stay, followed by monitoring for complications, readmissions, mortality, and relevant medical history for 3 to 6 months post-discharge. Participants provide consent and demographic, clinical, and surgical data will be collected and stored securely in hospital electronic records. The study tracks adherence, participation, and patient outcomes to help improve future protocols and policies. The primary outcome is establishing a database within one month, with secondary outcomes including in-hospital records and patient follow-up over 3 to 6 months, aiming to enhance care for frail surgical patients.

Participants are screened for eligibility to participate in the trial.
1 visit (in-person or online)
Duration - Up to 4 weeks before surgery
Participants undergo comprehensive assessments including frailty, cognitive function, functionality, cardiovascular risk, comorbidities, nutritional status, pain, and physical performance evaluations conducted by a nurse navigator and medical consultation.
1 outpatient medical consultation (in-person)
Duration - Variable, up to the time before surgery
Participants receive kinesiology evaluation and intervention sessions, and nutritional interventions tailored to their physical-functional and nutritional-metabolic status.
8 to 10 kinesiology sessions and 1 to 2 nutritional intervention sessions
Duration - Up to surgery
If necessary, participants may have additional contact via email, telephone, or telemedicine to discuss examination results and determine if a second consultation is needed.

Duration - Duration of hospital stay
Participants are monitored during their hospital stay for surgical complications, daily care level, and hospitalization duration until discharge.
Daily in-hospital monitoring until discharge
Duration - 3 to 6 months
Participants are followed up for 3 to 6 months after discharge to monitor complications, readmissions, mortality, and relevant medical history.
