Eligibility Criteria

Eligible

You may qualify if you...

  • Age greater than 18 years old
  • Living within approximately 20 kilometers of Gap, Laragne-Montéglin, Veynes, or Clermont-Ferrand
  • Invitation to participate extended by caregivers from medical structures
  • Diagnosed with a chronic illness covered by CNAM medical criteria such as stroke, peripheral arterial disease, rhythm disorders, valvular or congenital heart disease, stage I or II heart failure, severe hypertension, or chronic coronary disease
Not Eligible

You will not qualify if you...

  • Heart failure classified as stage III or IV according to NYHA
  • Contraindication to physical activity
  • Living in an institution
  • Unable to walk more than 100 meters independently
  • Diagnosis of Alzheimer's disease or other dementias per CNAM criteria
  • Cognitive impairment affecting comprehension
