Eligibility Criteria
You may qualify if you...
- Age greater than 18 years old
- Living within approximately 20 kilometers of Gap, Laragne-Montéglin, Veynes, or Clermont-Ferrand
- Invitation to participate extended by caregivers from medical structures
- Diagnosed with a chronic illness covered by CNAM medical criteria such as stroke, peripheral arterial disease, rhythm disorders, valvular or congenital heart disease, stage I or II heart failure, severe hypertension, or chronic coronary disease
You will not qualify if you...
- Heart failure classified as stage III or IV according to NYHA
- Contraindication to physical activity
- Living in an institution
- Unable to walk more than 100 meters independently
- Diagnosis of Alzheimer's disease or other dementias per CNAM criteria
- Cognitive impairment affecting comprehension
