Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age greater than 18 years old
- Living within approximately 20 kilometers of Gap, Laragne-Montéglin, Veynes, or Clermont-Ferrand
- Invited to participate by caregivers from medical centers
- Diagnosed with a chronic condition covered by CNAM medical criteria such as stroke, peripheral arterial disease, rhythm disorders, valvular or congenital heart disease, stage I or II heart failure, severe hypertension, or chronic coronary disease
You will not qualify if you...
- Heart failure classified as stage III or IV according to NYHA criteria
- Any contraindication to physical activity
- Living in an institution
- Unable to walk more than 100 meters independently
- Diagnosed with Alzheimer's disease or other dementias as per CNAM criteria
- Having an unclassified cognitive impairment that limits understanding
