What this Trial Is About
Researchers are studying how two different weight-loss approaches affect heart and metabolic health, along with molecular changes in adults with obesity. The study compares adults starting a GLP-1 receptor agonist medication semaglutide or tirzepatide prescribed by their own doctor against those following a structured lifestyle program involving a reduced-calorie diet and increased physical activity. This pilot study aims to examine improvements in health markers, appetite hormones, mental health, and gene expression. One group receives semaglutide or tirzepatide as prescribed by their medical provider, with dosing determined by the providers clinical judgment. The other group participates in a lifestyle intervention targeting a 500 kcal daily calorie deficit and 150 to 300 minutes of moderate physical activity per week. Lifestyle participants receive three individual counseling sessions delivered in person or via secure video. Both groups attend two assessment visits at baseline and after three months. Participants undergo measurements including weight, body composition, blood pressure, and resting metabolic rate. Blood samples are collected to assess cholesterol, blood sugar, insulin, appetite hormones, liver and kidney function, gene activity, and metabolites. Participants complete online questionnaires on diet, eating behaviors, stress, mental health, and quality of life. Lifestyle participants use apps or food journals to monitor diet and activity. Heart rate, sleep, and oxygen saturation are tracked with a wearable device for two weeks after baseline. The study monitors changes over three months to evaluate the effects of the two weight-loss strategies.
