Eligible

You may qualify if you...

  • People aged 60 or above
  • Living within the service areas of respective community centers
  • Cognitively competent with Hong Kong version of Montreal Cognitive Assessment (HK-MoCA) with score 2 or higher
Not Eligible

You will not qualify if you...

  • Unable to communicate
  • Not reachable by phone
  • Not living at home
  • Bed bound
  • Serious mental illness requiring hospitalization in recent 6 months
  • Already engaged in similar structured health or social programs
  • Will not stay in Hong Kong for the current three months
