Participants include clinicians and African American patients aged 60 or older with serious illnesses such as cancer, cardiac or pulmonary disease, neurodegenerative conditions, and others, as well as bereaved family members and pastors. Data collection includes patient and family surveys before and after the intervention to measure perceptions of therapeutic alliance and goal-concordant care. Researchers will also assess clinicians’ cultural knowledge, awareness of bias, and confidence in applying these principles. The study will continue through 2028, with outcomes monitored up to three months before and after training.

Eligibility Criteria

Eligible

You may qualify if you...

  • Self-identified African American (born, raised, and lived primarily in the US)
  • Non-Hispanic or Latino
  • Age 60 years or older
  • English-speaking
  • Diagnosed with a serious illness such as cancer, cardiac disease, pulmonary disease, neuro-degenerative disease, renal disease, stroke, sepsis, or hepatic disease
  • Callahan Six-Item Screener score of 4 or higher
  • Able to complete baseline surveys
  • Bereaved family members eligible if loved one died in the last 12 months with qualifying illness
  • Pastors eligible if self-identifying as African American or White, Non-Hispanic or Latino, age 18 or older
  • Clinicians practicing at UAB or Montefiore/Einstein sites with at least 3 months of clinical practice prior to training
  • Family members aged 18 years or older, English-speaking, community-dwelling, and unpaid care providers
Not Eligible

You will not qualify if you...

  • Currently receiving hospice care (persons with serious illness)
  • Paid "sitters" (bereaved family members and family member caregivers)
  • Lack of at least 3 months of clinical practice at the study site prior to intervention training (clinicians)
