Researchers are evaluating a metaverse-based surgical simulation platform designed to assist extracranial-intracranial EC-IC bypass surgery in patients with moyamoya disease or chronic cerebral ischemia. This study aims to assess how accurately the platform can locate key blood vessels before and during surgery. It also explores whether the technology helps enable less invasive surgery and improves safety. The study uses the NTU OpVerse surgical simulation platform, which combines medical imaging, 3D reconstruction, digital-twin technology, and extended reality including virtual, augmented, and mixed reality. Patient CT or MRI scans are converted into interactive 3D anatomical models for surgeons to review and plan the operation. During surgery, extended reality technology guides precise positioning of donor and recipient arteries. Participants will have their preoperative CT images uploaded to the platform, with intraoperative verification of vessel localization on surgery day. Researchers will measure localization accuracy, time to locate arteries, surgical durations, incision sizes, and postoperative complications up to 30 days after surgery. Other assessments include blood flow after bypass, neurological function changes, ICU stay length, and total hospital stay, providing comprehensive monitoring from planning through recovery.
