This observational study focuses on adult patients admitted to intensive care units after major surgery to evaluate the risk of needing reintubation, which can lead to complications and longer hospital stays. The study specifically examines postoperative patients who have been extubated in the operating room and are breathing spontaneously upon ICU admission. Researchers aim to understand how ultrasound measurements of respiratory muscles, lung ultrasound scores, the ROX index, and perioperative fluid balance relate to the risk of reintubation. Participants will undergo ultrasound assessments of diaphragm thickness, parasternal intercostal muscle thickness, and lung status within the first 0 to 2 hours after ICU admission. The ROX index will be calculated from oxygen saturation, inspired oxygen concentration, and respiratory rate. Researchers will also collect data on fluid balance during surgery and postoperative ICU stay. No new treatments or interventions will be given as part of this study, and clinical care decisions remain with the ICU medical team. Patients will be monitored for 48 to 72 hours after ICU admission to see if reintubation occurs. The study measures include reintubation rates within 72 hours, respiratory muscle thickness, and lung ultrasound scores shortly after ICU admission. All clinical decisions, including reintubation, are made following routine care. Study participation involves data collection and ultrasound measurements without altering standard treatment, with the lead sponsor being Prof. Dr. Cemil Tacolu Education and Research Hospital Organization.
