What this Trial Is About
This research aims to study how two types of anesthesia, Total Intravenous Anesthesia TIVA and inhalational anesthesia, affect thinking and recovery after sleeve gastrectomy surgery in adults with obesity. The study focuses on cognitive functions after surgery, recovery times, and any mental changes caused by anesthesia methods or surgery. Researchers want to find which anesthesia method leads to quicker recovery, better patient comfort, and fewer changes in thinking abilities.
Patients undergoing sleeve gastrectomy will receive either TIVA, involving propofol and remifentanil infusions, or inhalational anesthesia with sevoflurane and remifentanil. Monitoring during surgery includes heart and oxygen checks, blood pressure, brain activity measures, and ventilation adjusted by ideal body weight. Both groups will have recovery times recorded, such as eye opening and extubation time, and cognitive tests will be done before surgery and at several points after surgery.
Participants will complete Mini Mental State Examination MMSE and Addenbrookes Cognitive Examination ACE tests the day before surgery and multiple times up to three months after surgery to assess cognitive changes. Researchers will collect clinical data during surgery and in the recovery room, including vital signs and complications. The main outcomes measured are postoperative delirium, delayed neurocognitive recovery, and postoperative cognitive dysfunction, evaluated at specific time intervals after surgery to monitor cognitive health and recovery.

Screening
Duration - 2 to 4 weeks
Participants are screened for eligibility to participate in the trial.
1 visit (preoperative assessment)
Treatment
Duration - Day of surgery plus 3 months follow-up
Participants receive either total intravenous anesthesia or inhalational anesthesia during sleeve gastrectomy surgery. Cognitive tests are performed before surgery and at several times after surgery to evaluate thinking and recovery.
Multiple visits for cognitive testing at pre-surgery, hours 1, 6, 12, 24, days 3 and 7, and months 1 and 3 post-surgery
