Researchers are conducting a prospective cohort study involving adults with liver disease who are candidates for liver transplantation. The study aims to assess frailty, functional status, and disability using various functional measures. It also evaluates quality of life, personality, and cognitive function through questionnaires and cognitive tests. The purpose is to understand how frailty and physical function relate to outcomes before and after liver transplantation. Participants will undergo assessments including cognitive tests, quality of life evaluations, and physical performance measures such as grip strength, chair stands, balance, gait speed, and interviews regarding exhaustion and activity levels. Laboratory tests like creatinine, bilirubin, INR, sodium, and albumin will be collected from medical records. A subgroup of 1000 patients will provide blood, urine, stool, and tissue samples during transplant surgery for further study. Assessments will be done at baseline and at every clinic visit before transplant, continuing through 12 months post-transplant, then annually afterward. Researchers will track changes in frailty and physical function over time using the Liver Frailty Index. The study will monitor these measures to examine their association with transplant outcomes and overall patient health during follow-up periods lasting several years.

Duration - Ongoing throughout study participation
Participants undergo assessments of cognitive function, quality of life, personality, and measures of frailty and disability including grip strength, chair stands, balance testing, and gait speed.
Visits occur as participants are seen for liver disease treatment
Duration - During the course of treatment and surgery as applicable
Blood draw, urine, and stool samples are collected for a sub-group of participants. Additional tissue samples are collected during liver transplant surgery for this sub-group.
Sample collection occurs during clinical visits and surgery
Duration - Up to study completion on December 31, 2028
Participants are observed for changes in frailty, physical function, and liver transplant outcomes before and after liver transplantation.
Follow-up assessments occur during routine care visits
