Researchers are evaluating whether short social-media style health education videos can improve health care transition readiness, self-efficacy, emotional well-being, health literacy, and appointment attendance in adolescents with chronic illnesses. The study compares these videos to publicly available health education resources to see which approach better supports young patients during their health care transition. Participants will be randomly assigned to one of two groups. One group will watch seven short videos, each lasting 20 to 60 seconds, featuring adolescent volunteers teaching self-management and transition skills in a social media style. Videos are available in English or Spanish. The other group will use the GotTransition.org website, which offers youth-focused resources like videos and quizzes, also accessible in English or Spanish. Participants will access their assigned intervention for about 20 minutes. During the study, participants will complete surveys lasting 30 to 60 minutes immediately after the intervention and again at six months. Researchers will assess outcomes using questionnaires measuring transition readiness, self-efficacy, emotional well-being, and health literacy, along with medical record reviews to check appointment attendance before and after the intervention. The total study participation includes these follow-ups to monitor lasting effects.

Duration - Single session lasting up to 40 minutes
Participants engage with either the social media intervention consisting of 7 short health education videos or explore the GotTransition.org Youth and Young Adult Resources website. This stage focuses on improving transition readiness and self-management skills.
1 visit (virtual or in-person)
Duration - 6 months
Participants complete assessments immediately after the intervention and again at 6 months to evaluate transition readiness, self-efficacy, emotional affect, health literacy, and appointment adherence.
2 follow-up assessments (virtual or in-person)
