What this Trial Is About

Researchers are evaluating hemodynamic changes during bimaxillary orthognathic surgery by using data routinely collected during surgery. The study focuses on monitoring heart rate, blood pressure, mean arterial pressure, and oxygen saturation at different surgical stages. It also investigates the occurrence of the trigeminocardiac reflex TCR and its relationship with patient characteristics and surgical phases. This observational study takes place at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, K31r31kkale University. Participants undergo standard bimaxillary orthognathic surgery with continuous intraoperative monitoring. Hemodynamic parameters are recorded every 5 minutes throughout surgery, including stages such as anesthesia induction, incisions, bone cutting, osteotomies, mobilization, plate fixation, and suturing for both maxillary and mandibular procedures. The TCR is assessed by measuring decreases in heart rate and blood pressure, classified as mild or severe based on specific thresholds. No additional interventions or tests beyond routine clinical care are performed. During the study, participants intraoperative hemodynamic data will be analyzed to understand responses at each surgical stage. Researchers will compare patients who experience TCR with those who do not, considering demographic features and hemodynamic changes. The main outcome measured is the number of participants experiencing TCR during surgery, along with secondary outcomes like blood pressure, heart rate, and oxygen saturation. The study aims to improve early recognition and management of TCR. Participation lasts for the duration of the surgery with no extra procedures required.
