This research aims to evaluate how lateral crural steal combined with a columellar strut graft affects nasal tip projection and rotation in patients undergoing primary open rhinoplasty. Rhinoplasty is a common aesthetic surgery focused on reshaping the nose, with the nasal tip being a key and challenging area. The study addresses the need to preserve natural tip support and improve long-term outcomes by combining two surgical techniques to refine the nasal tip's shape and position. Participants will receive a primary open rhinoplasty procedure under general anesthesia. The surgery includes an open approach with specific incisions, correction of dorsal deformities, septoplasty, cartilage harvest, and cephalic trimming of lateral crura. The lateral crural steal technique repositions the lateral crus, while a columellar strut graft is inserted between the medial crura for added support. No extra grafts for projection enhancement are used. The incisions are closed, and nasal splints are applied after surgery. During the study, researchers will analyze nasal tip projection and rotation before surgery, immediately after, then at 6 months and 1 year post-operation. Patients will also complete satisfaction questionnaires before surgery, and again at 3 months and 1 year afterward. Follow-up visits will monitor the surgical outcome and patient satisfaction to assess the long-term stability and effectiveness of the combined surgical techniques.

Duration - Up to 1 year postoperatively
Participants attend follow-up visits to assess nasal tip projection, rotation, and satisfaction after surgery.
Visits immediately postoperatively, at 3 months, 6 months, and 1 year
