What this Trial Is About
Researchers are evaluating a 9-session cognitive-behavioral group intervention called Changing Lives and Changing Outcomes-9 (CLCO-9) for patients with serious mental illness (including depression, bipolar disorder, and psychotic disorders) who have current or past criminal legal involvement. This study takes place at Worcester Recovery Center and Hospital (WRCH) and aims to improve mental health and reduce legal risks by targeting both symptoms and risk factors for future legal involvement. The study also explores changes in clinicians' knowledge and attitudes about treating legal risk factors. The intervention is delivered in group sessions led by trained WRCH clinical staff or trainees. The study has two phases: first to implement the CLCO-9 program and adjust for success, then to evaluate its effectiveness by comparing patient outcomes before and after treatment. All patients receive the CLCO-9 group treatment alongside usual hospital services. Treatment involves attending up to 9 group sessions, and the research team plans to include about 20 patients who complete at least 5 sessions. Participants will be assessed through self-reports on mental health symptoms, aggression, criminal thinking, attitudes, and knowledge of treatment content before and after completing the program, about 7 weeks apart. Behavioral data such as violence risk scores, restraint/seclusion events, and treatment engagement will be reviewed from hospital records. Clinicians leading groups will complete surveys to measure changes in their knowledge and attitudes. Patient satisfaction, session fidelity, and engagement including homework completion will also be monitored. The total study duration is expected to be about 6 months from recruitment start to study end.
