What this Trial Is About
This trial focuses on patients undergoing coronary artery bypass graft CABG surgery with cardiopulmonary bypass. It evaluates the use of the erector spinae plane ESP block, a newer anesthesia technique, to manage postoperative pain. The study compares the effects of a local anesthetic solution versus a placebo in this surgical setting, aiming to provide clearer information about the ESP blocks use and benefits in cardiac surgery. Participants receive an ESP block after anesthesia induction, with either a local anesthetic ropivacaine or placebo saline administered bilaterally at the fourth thoracic vertebra. A catheter is placed on each side and connected to a patient-controlled analgesia PCA pump delivering either ropivacaine or saline continuously at 4 mLhour with bolus doses of 6 mL upon patient request. Standard multimodal analgesia including dipyrone, paracetamol, and tramadol is provided, with morphine available as rescue medication. PCA use continues until the fifth postoperative day. Participants are closely monitored for total morphine consumption from surgery completion until postoperative day five, which is the primary outcome. Secondary measures include chronic pain scores at 30 and 180 days after surgery. The study uses randomization and blinding to compare the two groups. Safety and effectiveness of pain control are assessed through clinical evaluations, medication usage, and pain scoring during hospitalization and follow-up visits.
