Researchers are evaluating the feasibility of implementing a Survivorship Care SC Plan for patients with hematologic cancer who undergo hematopoietic stem cell transplantation HSCT. This study focuses on whether this intervention can be successfully integrated within the Italian national health system. It aims to examine retention rates, quality of life, unmet needs, and the burden on caregivers, addressing the long-term challenges faced by patients and their families after HSCT. The intervention involves a personalized SC Plan tailored to each patients needs, including treatment summaries, follow-up recommendations, and advice on symptom management and health promotion. After hospital discharge, patients are supported by a care coordinator who evaluates their quality of life and perceived needs. Based on these assessments, the SC Plan is adjusted with referrals to educational resources and services available in the hospital or community. Participants are followed from hospital discharge up to six months after transplantation. Assessments occur at baseline, 100 days, and six months, including quality of life evaluations using the EORTC QLQ-C30 and EORTC QLQ-HDC29, caregiver burden via the Caregiver Reaction Assessment CRA, and perceived patient needs. The study also examines the usability of data collection from both patient and provider perspectives and the ability of providers to deliver the SC Plan over 12 months.
