Tobacco users and those with high alcohol consumption are referred to specialized care. The study measures changes before, at 16 weeks, and at 12 months. During the study, participants' blood glucose and plasma triglyceride levels, physical activity, and intake of healthy foods are monitored at baseline, 16 weeks, and 12 months. Additional assessments include waist circumference, blood pressure, HDL-cholesterol, and BMI over the same periods. Participants will be followed for up to one year to evaluate how these interventions affect their metabolic health and lifestyle habits.

Screening

Outpatient Treatment

Duration - 16 weeks

Participants receive health counseling focusing on physical activity and diet and have access to a digital platform supporting healthy lifestyle adoption.

Regular visits during the 16 weeks of counseling and platform use

Follow-up

Duration - Up to 12 months after treatment

Participants are monitored to evaluate the lasting impact on metabolic risk factors and lifestyle changes after the treatment period.
