Comparing Rifampin-Free and Rifampin-Containing Treatments for Staphylococcal Prosthetic Valve Endocarditis A Phase 3 Multicenter Randomized Study Evaluating Safety and Mortality Outcomes

Researchers are comparing two antibiotic regimens for treating staphylococcal prosthetic valve endocarditis, a serious heart valve infection caused by staphylococcus bacteria. The study aims to show that a regimen without rifampin is not worse than the standard regimen containing rifampin in terms of death rates within 6 months after starting treatment. This is a multicenter, randomized controlled trial designed to answer this important question because rifampin use can cause side effects and drug interactions in patients with prosthetic valves. Participants will receive one of two treatment plans either a rifampin-free antibiotic regimen or a rifampin-containing regimen with 900 mg per day, both following current European and French guidelines for endocarditis treatment. The choice of other antibiotics in the rifampin-free group is guided by these recommendations but is left to the treating physicians. The trial compares these two approaches to evaluate if avoiding rifampin reduces risks without increasing mortality. During the study, participants will be monitored for up to 12 months, with the main focus on all-cause mortality at 6 months. Researchers will also track other outcomes such as infection relapse, microbiological failure, embolic events, heart valve surgery, clinical failure, adverse events, bleeding, hospital stay length, antibiotic treatment duration, and healthcare readmissions. Participants will undergo regular evaluations including blood cultures and clinical assessments to measure these outcomes and ensure safety throughout the trial.
