The study is expected to continue until 2027.

Eligibility Criteria
You may qualify if you...
- Age 21 years or older
- Diagnosed with at least one chronic disease as documented in medical records
- Living in the community (not hospitalized)
- Fluent in Hebrew
You will not qualify if you...
- Documented cognitive impairment
- Diagnosis of major depression or other psychiatric disorder
- Hospitalization in the past 3 months

Participants are screened for eligibility to participate in the trial.
Duration - 6 weeks
Participants take part in a 6-week online group-based self-management support program with weekly 2.5-hour sessions led by trained facilitators.
Weekly online sessions
Duration - 6 months post-intervention
Participants complete assessments immediately after the program and again six months later to evaluate patient activation, health behaviors, symptom burden, and quality of life. A subset participates in focus groups to share their experiences and suggestions.
