What this Trial Is About

Acute kidney injury is a serious condition affecting many patients in intensive care units, often requiring dialysis called renal replacement therapy (CRRT). This therapy cleans the blood by removing toxins and excess fluid using a catheter connected to a dialysis machine. Doctors are uncertain about the best intensity of this therapy, as higher doses remove more toxins but also important nutrients and medications, and current practices vary widely. This research evaluates whether a lower intensity of continuous renal replacement therapy is as effective and safe as the usual dosing levels. Participants will be randomly assigned to receive either a lower dose of CRRT at 10-15 ml/kg/hr or the standard dose determined by their clinical team. Other treatments will remain unchanged. The study plans to recruit 20 patients in the UK and share anonymized results with a related Canadian study. Participants will undergo CRRT with monitoring of the total delivered effluent rate over 7 days to assess treatment intensity. Researchers will also evaluate the feasibility of the protocol over 12 months. No masking is used, and the study involves standard assessments during treatment. The trial may last up to three years from the start date, and participants' safety and treatment responses will be carefully observed throughout.
