The total follow-up period is designed to extend up to 50 years, with ongoing monitoring to evaluate long-term effects of the interventions.

Eligibility Criteria

Eligible

You may qualify if you...

  • Men and women aged 25 years or older
  • Have two or more of the following risk factors: BMI ≥24 or BMI ≤18.5 kg/m2, smoking, less than 30 minutes of active exercise daily, blood pressure above 130/85 mmHg, dyslipidemia, fasting glucose above 6.1 mmol/l, fatty liver
  • Patients diagnosed with hypertension, diabetes, or atherosclerotic cardiovascular disease (ASCVD)
Not Eligible

You will not qualify if you...

  • Patients who have not signed the consent form
  • Patients with severe heart failure, liver failure, kidney failure, or malignant tumors
  • Patients planning pregnancy

Outpatient Treatment

Participants receive lifestyle and health education interventions to manage risk factors of cardiovascular and cerebrovascular diseases, diabetes, and other chronic conditions. The intensive intervention group receives personalized management with health education interviews, calls, and online communication, while the standard intervention group receives general health education without active management.

Multiple contacts including 2 health education interviews, 4 calls, and approximately 50 online communications per year for the intensive intervention group
