Quality of life and economic burden on health are also assessed. The study includes regular data collection through surveys and health management networks established via internet technology. The total follow-up period is designed to extend up to 50 years, with ongoing monitoring to evaluate long-term effects of the interventions.

Eligibility Criteria
You may qualify if you...
- Men and women aged 25 years or older
- Have two or more of the following risk factors: BMI ≥24 or BMI ≤18.5 kg/m2, smoking, less than 30 minutes of active exercise daily, blood pressure above 130/85 mmHg, dyslipidemia, fasting glucose above 6.1 mmol/l, fatty liver
- Patients diagnosed with hypertension, diabetes, or atherosclerotic cardiovascular disease (ASCVD)
You will not qualify if you...
- Patients who have not signed the consent form
- Patients with severe heart failure, liver failure, kidney failure, or malignant tumors
- Patients planning pregnancy
