Eligibility Criteria

Eligible

You may qualify if you...

  • Child with any primary cancer diagnosis (non-relapsed, non-secondary malignancy) who has received cancer-directed therapy including chemotherapy and/or radiation therapy
  • Child is within specific time frames after chemotherapy or radiation treatment completion, up to 12 months
  • Child has one or more cardiovascular risk factors such as high body mass index, blood pressure, cholesterol, glucose metabolism issues, or specific cancer treatment exposures
  • Child is 18 years of age or younger at enrollment, or over 18 but under medical guardianship
  • Parent or guardian has screened positive for food insecurity using a validated 2-item screen
Not Eligible

You will not qualify if you...

  • Patient received only surgery or watchful waiting/surveillance as cancer-directed therapy
  • Family is a foreign national receiving cancer care as an embassy-pay patient
