What this Trial Is About
Researchers are evaluating a personalized, intense cardiac rehabilitation program called MICROFIT, which combines high intensity interval training HIIT and dietary advice, for patients with microvascular coronary dysfunction. This feasibility study aims to assess recruitment, retention, adherence, and acceptability of the intervention while gathering preliminary data on its effects on angina symptoms using the Seattle Angina Questionnaire. The trial also studies the clinical effectiveness and fidelity of MICROFIT compared to usual care. Participants will be randomly assigned to either the MICROFIT plus usual care group or the usual care group alone. MICROFIT involves a 24-week lifestyle intervention with phases including induction, consolidation, and maintenance, featuring one-on-one supervised HIIT sessions and home exercise alongside personalized dietary sessions focused on a Mediterranean-style diet. Usual care includes clinical review, risk factor management, lifestyle advice, and medication as needed according to guidelines. Both groups undergo detailed assessments at baseline and six months, with additional visits and interviews for the MICROFIT group. Throughout the study, participants will complete tests like cardiopulmonary exercise testing, echocardiograms, cardiac MRI, blood tests, and body composition measurements. They will wear activity trackers and keep diet diaries at the start and end of the study. Questionnaires will assess angina symptoms, mental health, and quality of life. Adherence to exercise and diet sessions will be monitored, and qualitative interviews will explore participant and practitioner experiences. The total study duration is approximately six months, with safety and progress closely observed.
